Instructions for Completing Your ApplyTexas Application U.S. Graduate Admission Application

(valid for the Summer 2018, Fall 2018 or Spring 2019 semesters)

Please submit either a paper or a web application, not both.

Biographical Information

  1. Enter your Social Security number. A Social Security Number is strongly encouraged for U.S. Citizens and Permanent residents. Please enter your Social Security Number if you have one as it is important for purposes of the identity of your application, grade transcript, test scores, and financial aid.
  2. Enter your date of birth (month, day, year). Do not leave blank; this information is required.
  3. Enter your full, legal last, first, and middle name. Do not use nicknames or abbreviations because this information will be used for your official record if you enroll. Use your full, legal name on all documents sent to the institution to which you are applying.
  4. Enter other names, if any, under which your transcripts or other credentials may be listed.
  5. Enter the city, state, and country where you were born.
  6. Answer the questions regarding your citizenship status. If you have one, be sure to include with your application a copy of your permanent resident card and/or notice of action from the USCIS.
  7. Provide the information regarding any U.S. military affiliations that you have.
  8. Enter your permanent mailing address. If you are filling out a paper application, use the county code only if you are residing in Texas; if you are not residing in Texas, you do not need to provide a code. If you do not live in the United States, enter the name of the country where you reside.
  9. Enter your physical mailing address , only if different from your permanent mailing address.
  10. Enter your preferred phone number and alternate phone number (if applicable). Enter the phone type for the preferred and alternate phone, if applicable (Home, Work, or Cell). Enter the country code, if applicable.
  11. Enter the name, relationship to you, address, phone number, and email address of an emergency contact person.
  12. Enter your email address. The inclusion of your email address may allow institutional representatives to contact you with admission information.
  13. Please indicate the highest level of your parents' or court-appointed legal guardians' educational background, and their relationship to you.
  14. Provide the information regarding your ethnic background and race. The information will be used for federal and/or state law reporting purposes and may be used by some institutions in admission or scholarship decisions.
  15. Designate whether you are male or female.

Educational Background

  1. Enter the name, city, state, and College Board or ACT high school code of your high school (please use the name search button for the most accurate information). Please enter the month and year you graduated or expect to graduate.
  2. If you did not or will not graduate from high school, please indicate whether or not you have earned a certificate of high school equivalency (like a GED) and whether it is the English or Spanish version.
  3. Enter any specific area of interest or specialty within your major. Check with the institution to which you are applying for more information.
  4. Degree you will seek. Check with the institution to which you are applying to determine which degrees are offered. Please indicate the ultimate degree you wish to seek at this institution.
  5. Please indicate whether you have ever been enrolled at or have previously applied to this institution.
  6. Please list the name of the institution (if any) that you are currently attending.
  7. Please provide a list of the college courses in which you are presently enrolled.

Educational Background continued

List ALL colleges and universities you have attended and any you plan to attend before the semester in which you hope to enroll. You must list all colleges attended even for a short period. Under "Degree," list any you have received or will receive prior to your enrollment. Beside the degree (in the same box) list your major; for example, BA in History. Be sure to include the date the degree was or will be awarded. If you have earned more than one degree from a college or university, please make an entry for each degree earned. The dates of attendance should be the same on each entry, but the degrees awarded should reflect the degrees and the actual date they were awarded.

If you need to list more than ten transcripts, please send a list of these schools, dates of attendance, and degrees earned or expected, along with your social security or student ID number and application ID to the university to which you are applying.

Please note that you will speed the processing of your application by searching for and selecting your college or university via our school search option. To begin the school search process, click the School Search button, then enter the name of your college or university in the box labeled "School Name". If you are unable to find a match in our database, please be sure to supply the city, state (if applicable), and country.

  1. Please list all post-secondary colleges and universities which you have attended or are currently attending, their school codes, their locations, the dates you were enrolled, the number of hours earned, any type of degrees you have received, and the date the degree was conferred. Include colleges you attended while you were in high school.
  2. Please indicate whether you are currently on academic suspension from any college or university.

Employment, References & Honors Information

  1. If you would like to be considered for a graduate assistantship or fellowship, please indicate by checking all of the appropriate boxes. Check with the program to which you are applying for availability and deadlines.
  2. EMPLOYMENT. Please provide complete information about EMPLOYMENT HISTORY.
  3. REFERENCES. List the names of  the individuals you have asked to recommend you, along with the organizations they are associated with, their positions at those organizations, and their email addresses.
  4. List all honors, financial assistance, awards, fellowships, scholarships, and assistantships received. If you need to list more than three awards, please send a list, along with your social security or student ID number and web application ID number directly to the appropriate admissions office at the university to which you are applying.

Essays

Some universities and graduate programs require essays or statements of purpose. You have two options for submitting your essays.

  1. Type your essay(s) or statement of purpose and mail to the university to which you are applying. Please put your name, Social Security Number and the Application ID at the top of each page. Check with the university to which you are applying for length and style guidelines.
  2. You may also submit your essay or statement of purpose on line. Use a word processing software to write your essay(s). When you are ready to save your essay(s) in Apply Texas, come back to ApplyTexas Main Menu page at https://www.applytexas.org/adappc/gen/c_start.WBX . Access your application, go to the Essays tab, and cut and paste your written, saved essay into the text area. You will be able to save, retrieve and edit the essay as many times as you want in Apply Texas prior to submitting your application.
